All domain name registrars go through the InterNIC, and domain names have to be renewed each year. Any registrars that offer services for multiple years are registering for you per year. You can choose a method of viewing the list of InterNIC registrars here: http://www.internic.net/regist.html.

Expect to pay $15 to $25 (US) for the domain name. There are domain name sellers who are not on the InterNIC listing, but they are buying from the registrar listing and reselling to you.

If you buy a web hosting package that includes a "free" domain name, make sure that the domain name belongs entirely to you and not the company in the contract. Otherwise, you may have to get the company's permission before switching web hosts.
